I, Philip J. rewer, a native and loyal citizen of the United State, hereby apply to the American Embassy at London for a passport for myself, accompanied by y wife Alice M., and minor children as follows: Alice M.C., born at London on the 10th day of October1911, 1, and....

I solemnly swear that I was born a Boston, in the state of Massachusetts, on or about the 2th, day of January 1883,1; that my father was a native citizen of the United States; that I am domiciled in the United states, my permanent residence being at Boston, in the State of Mass., where I follow the occupation of engineer that I left the United States in the month of January 1919, and am now temporarily sojourning a 2 Norfolk St. Strand; that I am the bearer of Passport No. 325, issued by this Embassy on the 11th, day of September 1905,1; that I intend to return to the United States within two years with the purpose of residing and performing the duties of citizenship therein; and that I desire the passport for the purpose of travelling to Spain.

I havenot applied for a United States passport elsewhere and been refused, and other countries in the interests of the Westinghouse Co.
